TPS accepting applications for transfers

Tulsa Public Schools is accepting applications for 2013-14 lottery, academic, interest and district transfers.

Transfer applications can be picked up in the Accountability office at the Education Service Center, 3027 S. New Haven Ave., or downloaded from the TPS website, under “Enroll or Transfer Your Child” under the “Parents” tab.

Applications must be returned to the Education Service Center by a parent or guardian no later than 4 p.m., Jan. 25, 2013. A parent’s or guardian’s ID is required to submit the application. Mailed applications and applications returned to school sites will not be accepted.

Transfer applications must include:

•current proof of address

•birth certificate for students new to TPS

•immunization records for students new to TPS

•guardianship papers (if applicable)

•sealed teacher recommendation (academic schools only)

•cumulative record/transcript (for students not currently enrolled or new to TPS)

•first semester report card (for students new to TPS)

•explanation of excessive absences (if applicable)
